1. Introduction
These Terms and Conditions (the “Terms”) govern your use of MyCare Companion (the “App”) during our MVP pilot. By creating an account or using the App, you agree to these Terms.
2. Who We Are
The App is operated by Silver IQ Ltd, registered in England & Wales (Company No. 16282310), trading as “MyCare Companion”. The App is offered as part of a limited pilot with participating NHS hospitals.

For patient support, email support@silveriq.net. For hospital or pilot enquiries, email pilot@silveriq.net. We will communicate any material updates to these Terms in‑app or by email.
4. Who Can Use the App
You must be 18 years or older. Access is by invitation or access code from a participating hospital. The App is provided free of charge to eligible patients during the MVP.
5. What the App Is (and Is Not)
The App offers supportive tools such as symptom logging, reminders, and an information hub to help you engage with your care. It does not provide clinical, diagnostic, or treatment advice and is not intended to be used in emergencies. Always seek advice from your clinician. In an emergency dial 999 or contact NHS 111.
6. Your Account and Responsibilities
You agree to keep your login secure, provide accurate information, and use the App only for your personal, lawful purposes. You must not misuse the App (for example, by attempting to gain unauthorised access, introducing malware, or using it for illegal activity).
7. Content You Provide
You remain the owner of information you enter. You grant the Provider a non‑exclusive licence to use that information solely to operate, maintain, and improve the App during the MVP. You confirm you have rights to upload any content you submit.
8. Data & Privacy (Summary)
We process personal data in accordance with UK GDPR and the Data Protection Act 2018. Further details are set out in our Privacy Policy, which explains what data we collect, our legal bases, how long we keep data, and your rights (including access, correction, deletion, and portability). For the MVP, Silver IQ Ltd acts as Data Controller for data collected via the App. Participating hospitals may receive anonymised, aggregated reports to help improve services. There is no automated decision‑making or profiling that has legal or similarly significant effects on you in the MVP.
9. Data Storage & Security
Data is hosted centrally in the UK with appropriate technical and organisational measures (including encryption in transit and at rest). We take reasonable steps to protect your information; however, no system is completely secure. If a personal data breach occurs, we will act in line with legal obligations, which may include notifying you and/or the ICO.
10. Service Availability and Changes
The App is provided on an ‘as is’ and ‘as available’ basis as part of a pilot. We may update or suspend features, or limit availability for maintenance or improvement. We may end the MVP at any time, giving reasonable notice where practicable.
11. Third‑Party Services and Links
The App may contain links to third‑party resources. We are not responsible for third‑party content or services. Your use of third‑party services is at your own discretion and subject to their terms.
12. Intellectual Property
All intellectual property in the App (including content, design, and software) is owned by Silver IQ Ltd or its licensors. You may use the App for personal, non‑commercial purposes only. You must not copy, modify, distribute, or reverse‑engineer any part of the App.
13. Ending Your Use
You can stop using the App at any time and request account deletion via the App or by contacting support. We may suspend or end your access if you breach these Terms, to protect users, or to comply with law or NHS guidance.
14. Liability
Nothing in these Terms excludes or limits liability where it would be unlawful to do so, including liability for death or personal injury caused by negligence, or for fraud. Subject to that, the App is provided without warranties and we are not responsible for indirect or consequential losses. We do not guarantee uninterrupted or error‑free operation.
15. Complaints
If you have a complaint, please contact us at support@silveriq.net. We will aim to respond promptly and work with your hospital team where appropriate.
16. Changes to These Terms
We may update these Terms to reflect improvements, legal or regulatory changes, or feedback from the MVP. We will notify you of material changes. Continued use after changes take effect means you accept the updated Terms.
17. Governing Law
These Terms and any non‑contractual obligations are governed by the laws of England and Wales. The courts of England and Wales will have exclusive jurisdiction.
18. Definitions
“MVP” means the limited pilot stage of the App. “Hospital” means an NHS organisation participating in the MVP. “Privacy Policy” means the document explaining how we process your personal data (insert link in the App).
